The Year in One Number: a drop rippling out with a reach pin on each ring

An impact report template is a layout for showing what changed because of your work: the one result that matters most, the numbers behind it, the people it reached, a voice from someone it helped, what the money paid for, and what you'll do next. Nonprofits send one to donors every year. Programs send one to their board or funder. Below are the six parts every impact report needs, a one-page template you can copy, and five impact report examples, from an animal shelter to a free clinic.

One‑Pager examples

Pinterest pin: an impact report leads with the change, 84% of families skip fewer meals, then reach of 96,000 meals in Q4, 72 cents of each dollar on food and one named voice

Every example was made with MakeOnepagers. Each one opens as a template you can edit for free: every word, chart and color.

What is an impact report?

An impact report tells the people who funded or approved your work what difference it made. It's different from an annual report, which covers the whole organization including finances and governance. An impact report focuses on results: who you reached, what changed for them, and how you know.

The best impact reports are short. A donor who reads one page and sees a clear result, a real person and an honest account of the money is more likely to give again than one who receives a 30-page booklet.

The 6 parts of an impact report

  1. The year in one number. The single result that sums up your impact. "84% of families say they skip fewer meals."
  2. Reach. How many people, places or hours, against last year. "96,000 meals in Q4, up from 62,000 in Q1."
  3. Outcomes, with evidence. What changed and how you measured it. "Survey of 600 families, September 2026."
  4. A voice. One short quote from someone your work helped, with their name or role.
  5. Where the money went. How each dollar was spent, ideally against last year.
  6. What's next. Two or three plans for next year, so the reader has something to support.

A one-page impact report template

SectionWhat to write, with an example
TitleWho and which year. Our Year: Riverside Food Bank 2026
The year in one numberThe main outcome. 84% of families skip fewer meals
ReachPeople or units served, with the trend. 309,000 meals, up each quarter
EvidenceHow you measured it. Survey of 600 families, September
A voiceOne short quote. "The Saturday box means my kids eat the same as their friends."
Where the money wentCents per dollar. 72 food, 14 delivery, 9 running, 5 fundraising
Next year2 or 3 plans. A second site in east Austin

Lead with the outcome, not the activity. Activity says how busy you were. Outcomes say whether it worked.

The template laid out on one page

A sample nonprofit impact report One-Pager made with MakeOnepagers: Our Year Riverside Food Bank 2026, an impact report for donors, the year in one number with 84% of families saying they skip fewer meals, meals served by quarter, 7 in 10 families with a child at home, where each dollar goes in 2025 and 2026, a quote from a parent and next year's plansEdit this template

The seven rows above as a donor sheet: one outcome made the hero, meals by quarter, who the families are, where each dollar went and what's next.

What to copy:

  • The year in one number. "Families skip fewer meals, 84%" is the outcome, with the survey that proves it right under it.
  • Where each dollar goes, two years side by side. 72 cents on food in 2026, up from 70. Donors see their money going further.
  • A named voice. "Maria, parent of two, Saturday pickup." One real sentence does more than a page of statistics.

6 impact report examples

Six reports for five readers: donors (twice), a school board, a foundation, a company's customers and team, and a clinic's board. Each keeps the six parts above in the order its reader needs them.

1. Nonprofit impact report for donors

A sample animal shelter impact report One-Pager made with MakeOnepagers: Second Chance Animal Shelter, Our 2026, with a 92% live release rate as the year in one number, adoptions, foster homes and spay and neuter counts, adoptions by month, an adopting family's words, where each dollar went and next year's goalsEdit this template

A shelter's year for donors: the one rate that sums it up, the numbers behind it, one family, the money and next year.

What to copy:

  • The rate against where you started. "92%, up from 81% in 2024." The trend is what tells a donor their gift worked.
  • One family, with a detail. "Biscuit had been with you for 140 days." A specific detail is more believable than a general thank-you.
  • Next year as numbers a donor can back. 400 foster homes, 95% live release, a second clinic.

2. Program impact report

A sample tutoring program impact report One-Pager made with MakeOnepagers: Tutoring Program Impact Report for five groups from September to June, what staff said, 78% of students gained a grade level, reading level before and after by group, and what to do next year with cost and ownerEdit this template

A sample program impact report: before and after for each group, the share who improved, what staff noticed and two changes for next year.

What to copy:

  • Before and after in one table. Reading level in September and June for each group, with the gain. The impact is the difference.
  • The group that didn't improve, explained. "The fifth is the group whose tutor changed twice." Honesty about the miss makes the 78% believable.
  • Next year's changes with a cost. "One tutor per group for the whole year, cost: none."

3. Grant impact report for a foundation

A sample foundation grant final impact report One-Pager made with MakeOnepagers: Read by Third, prepared for a family foundation, 71% of second graders reading at grade level against 44% at the start, four goals against results, $387 cost per child, the budget granted against spent, a teacher's words and what comes nextEdit this template

A final report to a funder: the result against the starting point, every goal against its target, and the budget as spent.

What to copy:

  • Goals against results, misses included. 148 tutors against a goal of 150 sits beside 620 children against 500. Funders check the misses first.
  • Granted against spent, line by line. Evaluation ran $7,400 over, and the table shows it.
  • What the grant leaves behind. "The district funds coordinators in 5 schools from 2027."

4. Company community impact report

A sample company community impact report One-Pager made with MakeOnepagers: Ridgeline Coffee Roasters Community Impact 2026, for its team, cafes and customers, with $3.40 a pound paid to 14 partner farms against a $1.92 market price, $186K donated, 2,900 staff volunteer hours, where the giving went, a grower's quote and goals for 2027Edit this template

A business reporting its giving to the people who buy from it and work for it.

What to copy:

  • The comparison up top. $3.40 a pound paid against a $1.92 market price says more than any mission line.
  • Where the money went, in four bars. Farm schools, food banks, job training and park cleanups, each in dollars.
  • A voice from the farm. One grower saying what the premium paid for.

5. Impact report for a board

A sample free clinic impact report One-Pager made with MakeOnepagers: Hope Street Free Clinic 2026, a report to the board of directors, with 3,420 patients and 11,860 visits, visits by service each quarter, an ER visit at $2,800 against a year of clinic care at $140, a patient's words and next year's plansEdit this template

A board report: the year's reach, where the work went quarter by quarter, and the cost it saved elsewhere.

What to copy:

  • Visits by service, stacked. Mental health visits rising each quarter is the trend a board needs to staff for.
  • The cost comparison. "$2,800 for one ER visit, $140 for a year here." It turns a clinic budget into savings for the county.
  • Next year with a fundraising target. "Raise $940,000, with 12% from new donors."

6. Donor impact report

A sample donor impact report One-Pager made with MakeOnepagers: What You Made Possible in 2026, Paws and Hope Animal Rescue's report for the 1,140 people who gave, with 1,312 animals adopted, 486 surgeries funded, an 18-day average stay and $842K raised, one dog's story, where each dollar went, this year against last and next year's goalEdit this template

A thank-you written to the givers: the year in four numbers, one animal's story, the money and what the next gift buys.

What to copy:

  • Talk to the donor, not about the charity. "What You Made Possible" puts the reader in the headline.
  • Price the next gift. "A monthly gift of $25 covers one emergency X-ray each month." A donor can act on a price.
  • This year against last, side by side. Adopted 1,040 to 1,312 shows the gifts working without a sentence of claims.

How to write an impact report in 6 steps

  1. Pick the one outcome that matters most. What changed for the people you serve? Make it the biggest thing on the page.
  2. Gather the evidence. A survey, before and after scores, or a comparison with people you didn't reach. Say how you measured it.
  3. Add reach, with a trend. How many, against last year or last quarter.
  4. Find one voice. A short quote from someone your work helped, used with their permission.
  5. Show the money. Cents per dollar, or spend by program, ideally against last year.
  6. End with next year. Two or three plans a donor or funder can get behind.

If your results are already in a spreadsheet, a grant report or a long annual report, MakeOnepagers reads the file and builds the one-page impact report from what's in it, which you then edit.

Impact report mistakes

  • Counting activity instead of change. "We ran 400 sessions" is activity. "78% of students gained a grade" is impact.
  • No evidence. An outcome without a survey, a test or a comparison is a claim.
  • Hiding what didn't work. Funders trust a report that explains a miss more than one with no misses at all.
  • No people. Numbers alone feel distant. One quote or one story brings them to life.
  • Too long. Most donors read one page. Put the detail in an appendix for the few who want it.

Frequently asked questions

What should be included in an impact report?

The main outcome in one number, your reach with a trend, the evidence for the outcome, a quote from someone you helped, how the money was spent, and your plans for next year.

What is the difference between an impact report and an annual report?

An annual report covers the whole organization, including finances, governance and leadership. An impact report focuses only on results: who you reached and what changed for them. Many nonprofits send donors the impact report and file the annual report.

How long should an impact report be?

One page for donors and supporters. A funder may ask for a longer report with methods and data, but even then, a one-page summary on top is what gets read first.

How do you measure impact for a report?

Measure the change, not the activity: before and after scores, a survey of the people you serve, or a comparison with a group you didn't reach. Say how many people the measure covers and when it was taken.

Is there a free impact report template?

Yes. Copy the seven-row template above, or open any of the examples on this page in MakeOnepagers and replace the figures with your own. Make yours free with the AI One-Pager Generator, and editing by hand is always free.
